Welcome to the Ages & Stages Questionnaires, Third Edition (ASQ-3) and Ages & Stages Questionnaires, Social Emotional (ASQ:SE) for children 0-5.    Below you will find links for screening your child.  Each screening looks for delays and difficulties in a variety of areas.  A therapist specializing in young children will review these screenings and call you  to review results and make recommendations.  If you have concerns you may call our office to schedule an appointment immediately.

These screens are free of charge for children and their families in San Bernardino County.  There are two stipulations:  You must register an address in San Bernardino County; and you must complete both the ASQ-3 and the ASQSE to be entitled to a free screening.

This is a screening instrument.  For serious concerns please see your pediatrician or local mental health provider immediately.
